How does CellTrak work?
When a client checks in, the CellTrak solution automatically downloads their electronic care plan, which they can view on their mobile devices and on the web. Staff can easily submit care plans on the web and on their mobile devices. When a client is released, the system will automatically notify field staff of any readmissions within the last 30 days, and the staff member who cared for the client will be notified as well.
